This is good information. I actually have a template I made in MS Word that is set up pretty much like this. I start every new draft out in the right format. It makes me feel more like it will actually be a complete manuscript someday.

I was glad you put where to put your real name and where to put your pen name. I had been wondering about that. I'm also curious about the process of choosing a pen name. Does anyone know anything about that? Are there legal issues at all? Do you just randomly pick one, declare it to be your alter ego and start writing it down? Are you obligated to list it as an alias from then on when you fill out official papers or background information? Advice?
